Tapestry Health offers state health insurance enrollment to Massachusetts residents. Please call (413) 582-0192 or any of our health services sites to schedule an appointment or to learn more information.

BENEFITS

• Preventative care checkups
• Care when you are sick or injured, including x-rays and labs
• Prescriptions for your local pharmacy
• Mental health and substance abuse treatment
• Vision care
• Dental care (for some members)

ELIGIBILITY

• You are at least 19 years of age (those who are younger may be eligible for MassHealth)
• You are a U.S. citizen or legal resident
• You cannot obtain health insurance from your employer
• You have an annual household income before taxes below 300% of the federal poverty level
