Clinician (NP, PA, CNM)- Holyoke, Springfield, Amherst

Join our team and make a difference! Tapestry Health is a non-profit organization servicing Western Massachusetts for 40 years.  We are currently seeking a dynamic candidate to fill the part time position (32 hours a week) of Clinician in our Holyoke, Springfield and Amherst sites.

Duties:

  • Perform routine initial and annual comprehensive exams for female clients
  • Perform STD screens for male clients
  • Review of client’s medical history
  • Order routine laboratory screening tests and additional laboratory tests as described in the protocol
  • Perform lab tests as needed and within scope of practice
  • Perform venipuncture as needed
  • Provide appropriate birth control methods in accordance with Tapestry medical protocols
  • Provide for the assessment and management of sexually transmitted diseases, vaginitis and urinary tract infections in accordance with Tapestry medical protocols
  • Make appropriate referrals to other health care providers for services beyond the scope of Tapestry
  • Attend staff meetings and in-service programs when possible
  • Attend the Medical Concerns Meetings and other pertinent meetings as scheduled
  • Act as medical consultant to staff at the health center
  • Perform other duties as assigned

Minimum Required Qualifications:  Licensed to practice in the Commonwealth of Massachusetts and Board Certified.  Demonstrates professional, positive attitude and ability to work with a diverse team.  Ability to serve diverse clientele and carry out administrative duties related to the practice.

Preferred Experience: Bilingual, Spanish.  Background in women’s health and sexual reproductive health

IT and Information Systems Coordinator - Florence

Tapestry Health, a non-profit healthcare agency serving western MA for over 40 years, seeks a full time Information Technology and Information Systems Coordinator to be responsible for all telecommunications and electronic data systems within the agency, including a new system of electronic medical records. Responsibilities include installing and maintaining computer hardware, software and networks. S/he will also manage consultants and vendors for the agency's information systems, networks, phone, internet accounts, etc. This position manages and provides hardware and software maintenance, training and consultation, and recommendations about future planning and development of resources.

Duties:

  • Plan, organize, direct, control and evaluate the operations of information systems and electronic data management, including the introduction and maintenance of a new Electronic Medical Records system.
  • Develop and implement policies and procedures for electronic data management and computer systems operations
  • Troubleshoot all technology/telecommunications issues
  • Troubleshoot hardware, software and network operating system
  • Control the telecommunications and computer systems budgets and expenditures
  • Meet with coordinators to discuss system requirements, specifications, costs and timelines
  • Hire and manage telecommunications and information systems as needed
  • Strong familiarity with all hardware and software used at the agency and new technologies available
  • Participate in orientation to new users of existing technology and train staff as needed to use the agency's technology products
  • Maintain current and accurate inventory of hardware and software as well as phone systems
  • Maintain log and/or list of required repairs and maintenance
  • Provide network access to all staff, appropriate consultants and interns
  • Install and configure work stations including the set-up of hardware and installation of software
  • Ensure network and data security
  • Manage telecommunications including ordering and installing equipment and analyzing use
  • Provide written documentation of all IT and MIS related processes
  • Perform other related duties as required

Essential Education, Experience and Skills:

  • Bachelor's Degree in Computer Science, Information Technology (IT) or Management of Information Systems (MIS). Master's preferred.
  • Work experience in a non-profit service environment preferred.
  • Candidates must have knowledge of a wide range of computer hardware and software systems and programs, computer networks, network administration and network installation, computer troubleshooting, viruses and security, and e-mail and internet programs
  • Candidates must demonstrate excellence in the following skills: ability to install and administer computer hardware, software and networks; team building skills, analytical and problem solving skills; effective verbal, written, presentation and listening skills; understand computerized accounting, spreadsheet, word-processing, database, billing and website development programs at a highly proficient level.
  • Ability to write basic code and work within the framework of above mentioned systems a plus.

Full Time Needle Exchange Harm Reduction Counselor - Holyoke

Join our team and make a difference! Tapestry Health is a non-profit organization that provides a range of health services throughout Western Massachusetts. Currently, we have an opening for a Full Time Harm Reduction Counselor at our Needle Exchange Program in Holyoke. Bilingual in Spanish is highly preferred.

Qualifications: Experience with substance abuse and knowledge of HIV/AIDS prevention principles. Knowledge of drug using culture. Valid MA driver's license and reliable transportation. Ability to be creative and flexible, work independently as well as in a team.  Skills in community education and organizing. Non-judgmental approach when working with clients.

High School Graduate or equivalent. Some working knowledge of the internet, Microsoft Word, and other computer programs. Knowledge of addiction theory, substance abuse treatment programs, HIV prevention, and viral hepatitis. Individuals in recovery are encouraged to apply.

Preferred Experiences, Skills, Education: Experience in street outreach. Ability to be creative and flexible, work independently and as well as in a team.  Experience with data entry. Individuals in recover strongly encouraged to apply.

Primary Responsibilities: The Harm Reduction Counselor is responsible for working as part of a team to provide outreach and education to injection drug users in regards to HIV prevention.  The Harm Reduction Counselor will work as part of a Harm Reduction Team to develop effective goals and strategies to identify, access, educate and support out-of-treatment injection drug users in reducing their risk for HIV infection or transmission. The Harm Reduction Counselor works as part of a team of Outreach Workers to ensure that services to the community are high quality, accessible and non-judgmental.  

Essential Duties:

  • Identify the injection drug using population through innovative outreach efforts.
  • Assess the education and prevention needs of IDUs and their sexual partners.
  • Develop contacts through area, including community leaders, agency staff, referral sources, business owners, and key nodes in local ‘grapevine”.
  • Place educational and prevention materials (e.g. condoms, bleach kits) in local outlets, and restock as needed.
  • Conduct presentations in various community sites, such as correctional facilities, drug treatment programs, bars, homeless shelters, and other gathering sites of targeted population.
  • Develop peer educators in target populations, and support selected educators with on-going contact.
  • Provide HIV counseling and testing according to CTSS guidelines.
  • Provide hepatitis A, B and influenza vaccinations to high risk populations in accordance to Tapestry Health (THS) policies and practices.
  • Conduct Needle/Syringe exchange according to THS Syringe Exchange Policies and Procedures.
  • Engage in one-to-one interactions, aimed at education and prevention, and referral to health services and substance abuse treatment.
  • Explain THS services and refer targeted population when appropriate.
  • Conduct health education session with targeted population.
  • Attend regular meetings of THS.
  • Complete reporting requirements in a timely manner.  
  • Supervise volunteers and interns as requested.
  • Accept other duties as requested.

Director of Clinical Services (NP, PA,CNM) – Florence

 

Tapestry Health is a non-profit health care agency that has been serving western Massachusetts for over 40 years. Founded as the Family Planning Council of Western Massachusetts in 1973, Tapestry remains the only non-profit organization in the region to offer family planning and reproductive health care to often marginalized individuals, such as young people, women living in poverty, recent immigrants, injection drug users, the homeless, and men and women with HIV/AIDS, regardless of their ability to pay. With offices in Hampden, Hampshire, Franklin, and Berkshire Counties, we are able to provide a variety of services to our clients including Health Services, WIC programs, HIV prevention and support, homelessness and re-entry programs, and we operate the only two needle exchanges offered in Massachusetts west of Cambridge.

 

Director of Clinical Services

 

The Director of Clinical Services will collaborate with the Medical Director and the Chief Program Officer to ensure that all clinical services are high quality and customer oriented. The Director of Clinical Services will provide clinical consultation and on call clinical services, supervise all clinical staff, conduct performance reviews of clinical staff and assist in the development and implementation of clinical protocol and procedures and agency Quality Assurance monitoring and compliance.

This 30-40 hour a week position will be based out of the administrative offices in Florence with travel to our locations in Springfield, Holyoke, Northampton, Amherst, Greenfield, and Pittsfield.

Duties:

 

  • Provide leadership and monitoring of the development of special projects and initiatives including implementation of new systems (clinical and administrative), pilot projects, and research projects. As well as the development and on-going implementation of meaningful use of EHR

  • Supervise clinical staff, to include the recruitment, training and on-going supervision.

  • Conduct annual performance reviews for clinicians

  • Conduct audits and chart reviews as mandated by MDPH, Title X, SAMHSA and other funders

  • Consult regularly with the Medical Director and collaborate on the development and implementation of medical protocols, medical policies & procedures, the drug formulary, quality assurance/quality improvement issues and new medical services and procedure

  • Act as on call medical consultant to all programs within the agency

  • Plan and conduct in-service training programs for licensed medical staff including the training and orientation of newly hired clinical staff including per-diem staff.  Oversee the training/monitoring of non-clinical staff in performing specimen collection including blood draws

  • Attend and conduct meetings as needed

  • Provide coverage at sites as needed

  • Oversee agency preceptorship initiative

  • Act as liaison between the agency and external vendors providing medical/laboratory services

  • Performs other duties as assigned

Minimum Required Qualifications: 

  • Licensed as an Advanced Practice Nurse in Massachusetts. (NP, CNM, PA)

  • Board Certified

  • Three year clinical experience required, women’s health preferred.

  • Able to sit at a desk to use computer and make phone calls.

  • Able to carry out typical administrative functions, computer skills.

  • Supports the mission and goals of the agency

  • Reliable transportation

Preferred Experience: Bilingual, Spanish.  Background in women’s health and sexual reproductive health.
